When someone loses their support structure at a moment of maximum risk — aging out of foster care, exiting incarceration, leaving a psychiatric hold, leaving a DV shelter, separating from the military, or losing housing — the gap that opens is operational. Siloed systems that can't see each other. Guardians of Solano is the coordination layer that closes that gap.
Solano County · 2024 data · District 3 pilot
Six populations, six funders, six systems that don't talk to each other. Guardians of Solano is one engine that connects them — starting with homelessness in Solano County.
