Federally-mandated outcome survey for foster youth at ages 17, 19, and 21
What it does
Outcome survey administered by states (via the Chafee program) to track foster youth wellbeing across financial self-sufficiency, education, employment, housing, and connection. Required at age 17 (full cohort), with follow-up at 19 and 21 (sample).
Eligibility
Foster youth in care at age 17 (baseline) and youth who participated in the 17-year-old survey, regardless of current foster status (follow-up at 19 and 21). Participation is voluntary.

Processing time
Survey takes ~30-45 min. Completing the NYTD does not by itself trigger services; however, many counties use NYTD completion as a touchpoint to re-engage youth with Chafee ETV, THP-Plus, and AB 12.
