Automatic Medi-Cal enrollment + targeted services in the last 90 days before release from jail or prison
What it does
CalAIM's Justice-Involved Initiative auto-enrolls qualifying inmates into Medi-Cal in the last 90 days before release and provides a defined set of pre-release services (care coordination, medication-assisted treatment continuity, MAT prescriptions for release day, durable medical equipment). Coverage activates on release day.
Eligibility
Adults incarcerated in CDCR state prison or Solano County jail with a release date within the next 90 days. Enrollment is initiated by CDCR or the Sheriff's Office in partnership with DHCS — the inmate does NOT self-apply.

Processing time
Auto-enrollment in last 90 days of sentence. Medi-Cal card and managed-care plan assignment are active by release day. The reentry person's MAT, prescriptions, and a 30-day medication supply are arranged in the days before gate release.
